The Gold Standards Framework (GSF)
The aim of the End of Life Care programme is to improve end of life care for all patients, irrespective of diagnosis and to provide greater choice for patients in their place of care and death. The North London Cancer Network is working towards this aim by leading on the implementation of the Gold Standards Framework in care homes across the 5 PCTs. Implementation of the Gold Standards Framework in GP Practices is being led by the PCTs.
Gold Standards Framework in Care Homes
The GSFis a framework to enable care homes to deliver a ‘gold standard’ of care for all people nearing the end of their lives. “It’s about living well until you die”.
The programme has 3 aims
1. To improve the quality of care provided for all residents from admission to the home.
2. To improve the collaboration with GPs, primary care teams and specialists
3. To reduce the number of hospital admissions in the final stage of life, enabling more to die with dignity in the home, if that is their wish.
There are 3 stages to the programme which run over an 18 month timeframe:
1. Preparation stage
2. Training stage
3. Consolidation and accreditation stage

Care Homes participating in the GSF programme
In 2007/08, 16 care homes participated in the GSF programme. 6 homes have applied to become accredited GSF care homes and will be informed of the outcome in June 09.
In 2008/08, 29 care homes are participating in the GSF programme.
